It's Not Too Late to Get Your Flu Vaccine

Flu Illness Activity is increasing in our area recently—it's not too late to get your flu vaccine and protect yourself!

Although no further Flu Clinics are currently scheduled, we still have plenty of vaccine available! If you want your child to receive the Flu Vaccine, please call our office at (413) 593-1333 to schedule an appointment.

Important Note for All Patients and Their Families

Our new EHR system requires that we obtain accurate demographic information. This will allow for effective insurance billing and allow us to ensure safety in medication prescribing, as well as many other things. We ask that you take the time to fill out this form prior to your visit to ensure that your visit is not delayed in any way. We appreciate your effort in being proactive in providing our office with your information, and encourage that you call our office with questions about the form.

Established in 1976, we are located in Chicopee, Massachusetts and we serve several communities in the Pioneer Valley and Western Massachusetts. Fairview Pediatrics is affiliated with Baystate Medical Center, and we accept most insurance plans.
